I acknowledge that if I am a youth leader or church leader and have completed this form on behalf of a person under the age of 18 years of age. I have done so with the permission and consent of the child’s parent/guardian and have been given the responsibility of caring for the child whilst at the conference and take full responsibility for the child.

If I am a parent/guardian of a child attending the conference, I hereby confirm and acknowledge my child/ward attends at their own risk and I allow this with full knowledge of the risks associated and confirm my child/ward is not the responsibility of Planetshakers, its staff, volunteers and agents.

Planetshakers recommends that you confirm your registration with our office prior to booking any flights and/or accommodation. Planetshakers does not accept any responsibility for losses incurred for flights and/or accommodation booked for the conference, including any budget accommodation provided by Planetshakers. Planetshakers reserves the right to change any of the published conference details without notice to you and have the final say on any matter relating to the conference.

I understand that when I purchase a number of tickets for any event that I must allocate each of these tickets to named individuals through the online system https://register.planetshakers.com. I understand that if I do not allocate any of the tickets that they will be forfeited and that no refund will be available for any unallocated tickets.

The event registration price does not include GST as the conference is considered a ‘Religious Service’. However, prices for any available ‘Add On’ items—e.g. for accommodation, catering etc—are inclusive of GST. Please retain the tax invoice that will be emailed to you for your records.

Planetshakers will only offer a refund or transfer of a registration to another Planetshakers conference/event if the conference is cancelled, rescheduled or significantly relocated (and you cannot or do not wish to attend the rescheduled or relocated conference), or to the extent otherwise required by law (including the Australian Consumer Law). Planetshakers will notify you of such changes as soon as practicable, and you will need to apply for your refund/transfer before the deadline notified to you or if there is none, within a reasonable time. Proof of purchase may be required for any refund or transfer request. No refunds can be provided after any stated deadlines or after the rescheduled conference/event date unless required by applicable law.

Planetshakers does NOT offer refunds or transfers to another Planetshakers conference/event for change of mind or changes in your personal circumstances (including not being able to allocate all group registrations). We provide the following alternatives to assist you: (1) Individual registrations can be transferred to another individual to attend the same conference by putting a request in writing to conference@planetshakers.com. (2) Group Leaders can reallocate registrations online through the online system https://register.planetshakers.com. No part registrations are transferable. Only registrations in their entirety are transferable.

Kids Conference is available for children aged 2 years to the last year in Primary School (at the time of conference). The parent/guardian of a child aged between 2–5 years must be a full-time delegate and must be in attendance at the conference whilst the child/ward is attending the Kids Conference. Parents/Guardians of children aged between 6 years to the last year in Primary School (at the time of conference) do not need to be in attendance at the conference, but are required to collect their children/ward during the breaks in between main sessions and during electives.

I hereby agree to inform the Kids Check-in Team of any custody issues that are in effect.

Where necessary, I agree that Planetshakers may authorise on my or my child/ward’s behalf whatever medical treatment that may be required (including, but not limited to, ambulance attendance and hospital treatment) at any time. I agree to pay all medical expenses incurred as a result. I understand that every effort will be made for the emergency contact(s) listed above to be notified before instituting such procedures.

If applicable, I consent to Planetshakers providing transport for me or my child/ward to and from the venue and activities as set out in the itinerary for the day and I understand Planetshakers will take all reasonably practicable steps to provide a safe environment for me or my child/ward and ensure all vehicles are in a safe and in working order.

I being the parent/guardian of the said child/ward hereby give my consent that my child/ward may participate in any activities they choose over the course of the conference, whether it is games, jumping castles, basketball, skateboarding, attending theme parks etc.

Where I have consented to participating or to my child/ward participating in activities at a local theme park, I hereby accept the inherent risks of possible injury or harm associated with the rides, shows and attractions at the respective theme park and, if applicable, the inherent risks associated with any activity in or near water.

I agree and understand that Planetshakers reserves the right to exercise its discretion to refuse to register any person upon medical and/or other grounds, without providing a detailed reason for doing so.

I agree to indemnify, release and hold harmless Planetshakers, its staff, volunteers and agents against any and all claims arising from, or in connection with, any injury, accident, misfortune, damage or loss that may occur to myself or my child/ward and/or my/their property, equipment or personal effects while present at the conference, within the conference premises and within any theme parks that are used for conference activities, including any injury that my child/ward may cause to another person.

I understand and accept that Planetshakers may collect and use information about me or my child/ward for the purpose of providing and offering promotional material and may provide the information to any third party who helps Planetshakers provide services to you. I consent to my personal details being used for the promotion of future Planetshakers events and products. The conference experience (as well as many other activities) will be captured in photographs, video and audio. Planetshakers reserves the right to use the material for promotional purposes. Images of people attending or participating in these activities may be used in our TV ministry, resources, and for other ministry or promotional purposes. I understand it is a condition of all registrants’ entry into the conference that all claims are waived in relation to inclusion of their likeness in such films or recordings. If I am a parent/guardian, I give my permission for my child/ward to be photographed or videotaped. I understand that the image may be displayed in the church publications, church buildings or websites. I understand that as a precaution my child’s/ward’s name will not be published or linked with photographs.

In signing this form, I acknowledge the registration information is true and correct and I have read, understood and agree to the terms and conditions as set above.
